Our concern for you as a patient does not stop just because you leave the hospital; we are also concerned with your wellbeing once you are discharged.

Developing a Plan
We will assist you and your family in planning for continuing any care that you need after dismissal from the hospital. A plan will be coordinated with the hospital provider, your physician and any other members of the health care team involved in your care.

Available Services
We can provide information and help in arranging a variety of services to aid in your recovery, such as skilled swing bed care, home care services, assisted living, care from nursing homes or long-term care facilities, Hospice and financial assistance. We can also refer you to the area agency for aging services.
