About Our Medical Social Services

  • Developing a Plan

    We will assist you and your family in planning for continuing any care that you need after dismissal from the hospital. A plan will be coordinated with the hospital provider, your physician and any other members of the health care team involved in your care. 

  • Available Services

    We can provide information and help in arranging a variety of services to aid in your recovery, such as skilled swing bed care, home care services, assisted living, care from nursing homes or long-term care facilities, Hospice and financial assistance. We can also refer you to the area agency for aging services.
